What is breast removal for trans people?

Breast gland removal in transgender (FTM, nonbinary) people is a surgical procedure that aims to reshape the chest to match the male appearance. It is an operation that removes the breast gland (mastectomy) and reshapes the chest, including the position or size of the nipples. The aim is to achieve a natural and aesthetic masculine chest appearance. Depending on the size of the breast tissue and the elasticity of the skin, several techniques are used (e.g. periareolar, double incision).

Consultation with an expert

  • Breast examination, assessment of skin elasticity
  • Selection of the appropriate technique (e.g. double incision, periareolar method)
  • Obtaining informed consent
  • Preparing for the operational plan

Preoperative examination

  • ECG, blood tests, internal examination – available at our clinic
  • Blood test results must not be older than 30 days prior to the procedure
  • It is recommended to stop using the binder before the procedure

Preparation before the procedure

  • Stop taking blood thinners 7 days before the procedure
  • Do not smoke 24 hours before the procedure
  • 12 h before the operation do not eat, drink alcohol or coffee
  • Do not use body creams and perfumes on the day of the procedure

Procedure

  • Duration of the procedure 1.5-3 hours
  • Removal of the mammary gland and excess skin
  • Nipple/chest adjustment as needed (position, size)
  • Drainage and elastic bandage

After the procedure

  • Hospitalization 1 day
  • Compression vest for 3-6 weeks
  • Regular checkups and dressing changes

Follow up

  • First check up after 7-14 days
  • The drains are usually removed the day after surgery
  • Monitoring scars and healing

Restrictions: one week of rest after the procedure

Recommendations

  • Wearing a compression vest as recommended
  • Sleep on your back for 2 weeks
  • Rest 1-2 weeks
  • Return to normal activities in 2 weeks
  • Sport in 4-8 weeks
  • Scar care (massage, silicone strips)

Contraindications

Possible complications

The treatment is not suitable for people who:

  • With active psychological or hormonal instability
  • In acute infectious disease
  • In case of insufficient skin elasticity without the possibility of removal

Possible complications are rare and may include:

  • Bleeding, infection, swelling
  • Seroma (fluid accumulation), nipple necrosis
  • Visible scars or asymmetries
