What is EVLA (endovenous laser ablation)?

The modern EVLA method uses a laser to remove damaged veins quickly, gently and with minimal recovery. EVLA is a minimally invasive procedure that uses the energy of a laser beam to close diseased veins. The laser causes selective heating of the vein wall, causing it to shrink and close. Over a period of 6-12 weeks after the procedure, the vein gradually scars and disappears.

EVLA is a gentle alternative to conventional surgery – with less pain, shorter recovery and better cosmetic results. The advantage is that it is performed under ultrasound control. The procedure is performed under local anaesthesia, when a diluted anaesthetic is injected into the vein area. This ensures low painfulness of the procedure and protection of the surrounding tissue from overheating. In some cases, it is possible to use a combination with analgosedation or general anaesthesia at the client’s request. The procedure takes 30-60 minutes.

Preparation before the procedure

  • In the case of local anaesthesia, the procedure can be performed without restriction of food and fluid intake (a light diet and sufficient drinking is recommended )
  • For general anaesthesia, do not drink, eat or smoke for 6 hours before the procedure.
  • It is advisable to provide an escort for the procedure.

Procedure

  • Lasts 30-60 minutes
  • Under ultrasound control, an optical fibre is inserted which seals the vein when it is pulled out.
  • The side branches are then removed using a small instrumentarium (mini-phlebectomy).
  • Outpatient procedure without the need for hospitalization

After the procedure

  • Putting on compression stockings
  • Short observation according to the condition and going home

Follow up

  • Clinical and ultrasound follow-up 6-8 weeks after the procedure.

Increased restriction: 1-2 weeks of rest after the procedure

Recommendations

  • Wearing compression stockings for 24 hours straight for the first 3 days
  • Another 10 days of wearing compression stockings 24 hours a day with a break for a shower, and 7 days only during the day
  • Walking possible immediately
  • After 7 – 10 days of light sports activity
  • After 3 weeks of strenuous physical activity and sport

Contraindications

Possible complications

The treatment is not suitable for people who:

  • With deep vein thrombosis
  • With acute inflammation of the veins
  • Pregnant women

Possible complications are rare and may include:

  • Bruising at the treatment site (absorption approx. 14 days)
  • Feeling of tension at the site of the sealed vein
  • Minor hardening after removal of varicose veins (absorption 6 – 8 weeks)
  • Very rarely superficial venous thrombosis or pigmentation
